Client Services and Case Management

• Develop therapeutic relationships and use reflection and motivational interviewing techniques with clients and families in a home visiting environment, modeling a client-centered, strengths-based, and solution-focused approach.

• Maintain a caseload of no more than 25-30 clients.

• Meet with clients and their families, in their home, in accordance with the NFP model and guidelines.

• Follow nursing processes in implementing NFP into nursing practice.

• Assess physical, emotional, social, and environmental needs of clients and their families as they relate to the NFP domains.

• Assist clients and their families in establishing goals and outcomes.

• Provide education, support and referral resources to assist clients and their families in attaining their targeted goals.

• Coordinate access to community resources.

• Consult and collaborate with other professionals involved in providing services to women and families.

• Formulate nursing diagnoses based on nursing assessment and client goals.

• Evaluate client progress toward stated goals and NFP outcomes.

• Plan home visits in accordance with client goals and NFP outcomes.

Supervision, Reflection, and Collaboration

• Meet with the NFP Nurse Supervisor weekly for clinical supervision.

• Use reflective practice during supervisory sessions.

• Schedule joint home visits with the NFP Nurse Supervisor every four months.  

• Attend and participate in weekly case conferences or staff meetings.

Data Entry, Evaluation, and Quality Improvement

• Ensure timely and accurate data input in the NFP Management Information System (MIS).

• Participate in review and analysis of the MIS reports to identify achievements and areas for improvement.

• Participate in quality improvement initiatives.

• Maintain confidentiality of all client and organizational information.

• Document appropriately.  Ensure timely and accurate data input and utilization of the web-based reports and other reports available through NFP.

Organizational Alignment and Representation

• Follow and support all policies, procedures, guidelines, and standards of NFP and HMB.

• Participate in community coalitions or meetings to support NFP and HMB initiatives.

• Promote public awareness of NFP and HMB as requested.

• Develop and maintain professional community relationships to support client referrals and partnerships.  

• Understand, support, and represent the NFP and HMB mission, vision, and model positively in actions and words to both internal and external customers and colleagues.

Teamwork and Communication

• Maintain a positive and collaborative work environment and experience for co-workers and clients.

• Consider differing viewpoints when analyzing issues and problem-solving.

Professional Development and Certification

• Engage in professional development to meet all NFP Nurse Home Visitor competency requirements

• Complete all required NFP education and training.

• Obtain and maintain current CPR certification.

• Asses personal learning needs and develop strategies to meet them.

• Use computers for distance learning as required.

Additional Responsibilities

• Work evenings and weekends as required.

• Perform other duties as assigned by the NFP Nurse Supervisor.

Associate of Science in Nursing (ASN) required.

Bachelor of Science in Nursing (BSN) preferred.

Must maintain a valid driver’s license and a vehicle for full-time business use. Employees must also maintain automobile insurance with minimum liability coverage of $300,000 Combined Single Limit (CSL) or $250,000 per person/$500,000 per accident.

Maintain current CPR certification.

Minimum of two years of nursing experience required.

Two years of recent experience in maternal, child health, or public health preferred.

Experience in home visiting or behavioral/mental health nursing preferred.

Public or community health experience highly preferred.

Demonstrate knowledge of pregnancy, labor and delivery, and infancy.

Demonstrate integrity and the ability to build trusting, collaborative relationships with coworkers, clients, and community partners.

Relate in a culturally sensitive manner to low-income clients and those from diverse backgrounds.

Experience with and a good understanding of Medicaid insurance preferred.
