Women's Wellness Initiatives Coordinator

Job Summary

Within Healthier Moms and Babies (HMB) Healthy Start Program and reporting to the Program Supervisor, the Women’s Wellness Initiatives Coordinator manages outreach and programming directed toward improving the physical, emotional and social health of low-income women before they become pregnant or become pregnant again to reduce the rates of poor birth outcomes and infant mortality in targeted zip codes within Healthy Start. Collaborate with the HMB home visitors to encourage participants to enroll in the program. Recruit participation of women in the program, plan, promote and facilitate activities to emphasize health and wellness. Promote community outreach awareness of the importance of women’s preventive health and wellness as it relates to improved birth outcomes. Serves as an organizational representative on health and wellness coalitions and other related groups to coordinate activities. Provide support and guidance to women in the program. Create awareness of the organization’s services which can assist women. Develop relationships with the program participants to encourage long term involvement with the program.

Experience

BA/BS Degree in Health and Wellness, Health Promotion, Human Behavioral Science, Public Health, Social Work or Human Services. Minimum of three years’ experience in health and wellness education and/or health promotion specifically working with low-income families preferred. Minimum of three years of case management experience. Knowledge of Fort Wayne’s community agencies and social service network preferred.
