Burmese Case Manager
Within Healthier Moms and Babies (HMB) Healthy Start and reporting to the Programs Manager, the Healthy Start Burmese Case Manager provides health education and case management services to low-income, high-risk pregnant women in Allen County, visiting them in their home with a goal of improving the outcome of pregnancies and reducing infant mortality. The Burmese Case Manager will provide translation services for the Nurse Family Partnership program on an as needed basis.
Must be fluent in Burmese and English. Minimum of two years’ experience required. Case Management, preferably with home visits, experience with maternal child health; familiarity with local social service delivery network preferred. Previous experience in case management, home health, maternal child health or other work directly related to the mission of HMB preferred.
