Now Hiring- Assessment Worker/Case Manager

Job Summary

Within Healthier Moms and Babies (HMB) and the Prenatal Home Visitation Program and reporting to the Program Supervisor, the Assessment Worker/case manager will be responsible for reviewing all referrals of prospective participants to determine eligibility for programs offered by HMB. Provides prenatal care coordination which may include scheduling first prenatal visit, arranging for Medicaid taxi and referrals to for extra supportive services if needed. The Assessment Worker/Case Manager will also provide health education and case management services to low-income, high-risk pregnant women in Allen County, visiting them in their home with a goal of improving the outcome of pregnancies and reducing infant mortality.

Minimum of two years’ experience required. Case Management, preferably with home visits, experience with maternal child health; familiarity with local social service delivery network preferred. Previous experience in case management, home health, maternal child health or other work directly related to the mission of Healthier Moms and Babies preferred. Must have a current valid Driver’s License.
